Maybe scope for improvement of bolt-plate design. One would think a 90degree would be less likely to come off. And a reason to standardise bolt size. Smaller heads may come off more easily. A good first year mechanical engineering design project, plus test to destruction. And route descriptions might note non-standard bolt size.
