Good idea, but may not stop duplicate posts on the topic, posts in wrong Forum section, etc.

Feel free to populate this sticky thread with relevant links ajfclark and others.

February 2009 Trango, BIG BRO # 5 Recall
It has come to our attention at Trango that one hundred #5 Big Bros made in March and October 2008 have improperly aligned inner tubes. Through an error in CNC machining, the inner sleeves on these units are rotated slightly to the side. This rotation causes the ends of the unit to be slightly out of parallel. Although barely noticeable in use, this off center orientation is not to spec and should be corrected. It could cause additional instability in some placements, particularly more parallel-sided cracks. As always, climber safety is our top priority, so, we are recommending that all recent #5 owners inspect their units, and if they are out of spec, return them to us at Trango for replacement of the inner tube. See below for instructions on identifying the problem. #5 Big Bros manufactured before March 2008 and all other sizes are not affected. We greatly appreciate your support in finding and returning the offending units to us. It is our highest priority to make the repairs and return them to you as soon as possible. Our manufacturer is already working on replacement inner tubes so that we can complete the repairs as we receive units back. These units are not batch marked, so the out of spec units must be identified by visual inspection. If the unit was delivered to the shop or purchased before March 2008, then it's fine. If it was delivered to the shop after March 2008, then it will have the flaw. If it was purchased after that date, but was from stock received prior to that, then it too is fine. If you're unsure of the purchase date, please check your Big Bro or call us at Trango.

FROM MIKE LAW

This biner was removed from the top of "These People are Sandwiches" at Shipley upper. It was a single point and looked like a nice stainless wiregate. An eagle eyed Victoria saw a crack (possibly it was more open under body weight) and retired it.
At this point it looked like a standard bent wire biner with an unnecessary weld in it.

I had assumed there would be some awesone weld defect and I could rabbit along about residual stress, and so i broke it with the gate open to preserve the fracture surface.
It broke at 169 kg. Now with the gate closed it would undoubtably be stronger, probabaly more like 350kg. Still not enuff I say.
Once I looked at the broken surface I got a shock, there was a lot of corrosion and some very big grains.

As a preliminary result, it looks like it's cast (weak, poor toughness, poor corrosion resistance, rapid fatigue) rather than being made from bent bar. A rough calc at 10 lowers a week showed it would have grown through wall in a year.
If you see one of these, use 2, or back up some other way. The plain bent bar unwelded variety should be ok. I'm going to section this and look a bit more closely

Petzl Scorpio Accident: http://www.petzl.com/en/outdoor/news/products-news-0/2011/05/13/information-regarding-via-ferrata-accident-involving-scorpio

Inspection notice: http://www.petzl.com/en/outdoor/news/products-news-0/2011/05/13/product-safety-alert-request-inspection-scorpio-via-ferrata

As a precautionary measure, Petzl has voluntarily decided to issue a product safety alert: request for inspection for the SCORPIO lanyards L60 and L60 CK manufactured before March 4, 2005 with individual product identification numbers between 02150 and 05062 for immediate inspection by Petzl and for return of those that pass inspection as rapidly as possible.

grigri2 recall

http://petzl.com/en/outdoor/recall-replacement-grigri-2

"Concerns all GRIGRI 2’s (D14 2O, D14 2G, D14 2B ) with the first five digits of the serial number between 10326 and 11136."

"Petzl has discovered that exerting excessive force on the fully extended handle of the GRIGRI 2 can cause internal damage, such that the GRIGRI 2 handle may become stuck in the open position.

When the handle is stuck in this position the assisted braking function is disabled. A damaged GRIGRI 2 in this configuration will function similarly to a manual belay device (e.g. tube style device).

When using a damaged GRIGRI 2 with the handle stuck in the position as shown in Figure 1, failure to control the braking side of the rope will increase the risk of an uncontrolled descent. A GRIGRI 2 with a damaged handle must be immediately retired from service."

Recall of EDELRID Klettersteig Sets

Not sure how relevant this will be in Oz but nonetheless:

Notice:  Recall of EDELRID Klettersteig Sets
On the 5th August, a fatal fall occurred on the Ottenalm-Klettersteig (difficulty C/D). A modern Klettersteig Set from the EDELRID company, (model Cable Lite - year of construction 2011) didn't withstand the burden of the fall -both load arms tore apart!

Michael Larcher was called in as expert by the public prosecutor's office Innsbruck. In close cooperation with the alpine police and the expert of the EDELRID company, the cause of the accident is now being intensively researched. All that can be said at present: An application mistakeby the 17-year old casualty (first supposition) can be excluded as well as the likelihood of safety limits.

Moreover, extensive tests with used sets of this construction (from other distribution places) so far indicate a hidden defect in the particular band construction of the elastic karabiner branches. Sets of this construction age especially quickly! This means that the shear/tear strength of the ribbons clearly already lies below the demanded normal values after relatively short use.

Old news regarding Alien Cams recall, put here for posterity / easy finding, if required in future.
January 12, 2006
Colorado Custom Hardware (CCH) has recently completed an investigation, and extensive testing of our product to identify and isolate safety issues concerning the brazing on CCH Alien cams. The safety of our customers is our number one priority and therefore we are issuing a recall for any cam bearing the marking described below.

The units with this issue are marked with a small centre punch dimple (see below) at the base of the round ball where the axle goes through the cable eye
. Although few failures have been reported, CCH recommends immediately discontinuing the use of any Alien cams with this mark. We are issuing a full recall of these cams.

Please return them to CCH for a free replacement. You can mail your cams to:
Colorado Custom Hardware Inc.

CCH has always worked to assure the absolute highest quality of every cam we produce, and we are currently working on ways to further enhance our already stringent standards of quality control. Again, I would like to reiterate that ensuring the quality and safety of all CCH products is our highest priority.
