alex, julian here, i realise that this is unsolicited advice so, needless to say, i offer it to you free of any obligation. the comments above are, for the most part, very good. any surgeon should send you away for rehab b4 they would operate, though not all will. some people alluded to exercises for your shoulder which are aimed at re-stabilising the gleno-humeral joint (the main joint in your shoulder). these are an excellent idea though most rehab professionals are not upto date with them. rubber band exercises can be effective though not always; these are what most therapists will prescribe. they help primarily with positional strength, but not much with proprioception which is the major component of preventing recurrent dislocation in over-head activities.
it sounds like yours was dislocated for a while; this is, shall we say, not so cool. it may be that you do little else about it and do not have a recurrence at all. however, there is a very high chance of subsequent dislocation.
if the rehab does not work then there are a few different types of operations which may help. these are a last resort only.
