I've dislocated my shoulder numerous times, in the forward direction (anterior dislocation). I'm not a physio, but I've done a couple of first aid courses, and from what I've heard this kind of dislocation can be caused by having an imbalance in ligament strength and flexibility between the front and back of the shoulder, and it's also the most common type of shoulder dislocation.
I've only seen one physio about it, about two years after it first happened. I haven't had an op done, as the physio recommended not to, or at least to put it off until no other option remains, as you can end up with your shoulder being more vulnerable than it was before. I've been doing exercises using a section of inner tube in order to strengthen the ligament behind the shoulder, and this seems to work well.
IMHO probably the safest thing to do would be to get a second opinion from another qualified physio, just to make absolutely certain before you commit to an op. Hope you have a speedy recovery!
