For many years i have used Buddha Treks in Kathmandu. They have a website (type in Buddhatreks) and are consistently very honest and caring. They have been in business for many years so aren't like the fly-by-night outfits that disappear and get rebadged in the same office. Remember to budget, whoever you get, for a tip at the end of the trek. A good way is to work out a daily charge (eg $30 a day) and the guide will pay for all accom, food within reason, bus fares. That way you don't have to think about costs along the way.
