I have heard (can someone enlighten us) that the taipan ones were screwed in till they jammed, then twisted out, hole cleaned, twisted in again, this was repeated a few times. Unfortunately you have a massive lever when using a spanner to twist them in and when they jammed they yielded in shear and the failed rapidly by SCC (stress corrosion cracking) just inside the hole a few months later.
