First step, ask to speak to the owner, not the manager. Make an appointment to do so if he/she manages in absentia. Dont waste their time, be prepared. Have a list of your concerns ready and ask them to go over some of the more major points in your presence.
If you have spoken to them and they have fobbed you off, put your concerns in writing and send it certified mail. In the letter ask for a written response to your concerns and allow them a reasonable period to do so, maybe 21 days from the date on your letter. In a non threatening way point out that if you donot receive a response you will raise your concerns with the appropriate authorities.
If you recieve no response to your letter, write a second one, this time to the council responsible for the area the business is located in. Outline your concerns and include a copy of the first letter you sent to the gym.
If the council dont manage that type of concern internally (some dont these days) then contact Consumer affairs in your state and provide copies of all correspondance, and outline your concerns. Assuming they are modelled like OCBA in SA they will have a Compliance an Auditing section, if not they will be able to put you in touch with the relevant state government department.
Its not generally Worksafe's core business to handle complaints of this nature, they will refer it to the Compliance and Auditing group, unless of course they are the compliance and auditing group in your state, which in some states they are.

I would be very surprised if you dont get something happening after the first letter.

You're probably going to be looking for somewhere else to climb too, you'll probably get struck off their christmas card list.
