Make sure you read the details of any private health insurance ambulance cover, as often they only cover you if the trip is entered by the ambos as an emergency (actual ambos please correct me if I'm way off the mark). If you need subsequent transport for treatment which is not classed as emergency transportation you could still be up for $$$. $60 is cheap IMO. Also for travel health insurance http://www.ihi.com/ seem pretty good: no exclusions unless you're motor-sport racing.
