Ive had it happen twice (truely stuck ropes that no amount of flicking and heaving will remove). Once on the recently removed rap down the Flutes at Ben Lomond. The cause then was mass twisting of ropes when pulling (pilot error, im now more careful), the solution involved a dodgy lead up to recover the end of the rope, and learning to prussic. The second time was on Bulging Buttress ont he Organ pipes, and was caused by the falling ropes when pulled looping over a rock spike and the end of the rope then getting wedged in a crack (bad luck). Solution involved re-leading the route. So neither time was the knot to blame.
