I have seen even the anchors unclipped in the situation Dave describes - and as it was a roof climb (the 18 at narrabeen slabs) he was too pumped/scared to let go with either hand to re-clip the rope into the anchor when we pointed out the problem!! Fortunately he was tied into the middle and had been backclipping the rope behind him for the third climber - so we quickly chucked him on belay on that rope!
