Brad Gobright, one of the most accomplished free solo climbers in the world, died today, Wednesday, November 27, in an accident in El Potrero Chico, Mexico.

A fair bit of confusion / possible conjecture in that link.
... He apparently abseiled off the uneven end of his rope, while simul-abseiling a pitch five or six pitches off the deck, even after his partner offered to equal-centre the rope beforehand...
No tail knots used either...

Sad to lose such a talented and out there bloke like that.
I feel the loss for his family and friends.

Complacency is a poor arrow in a climbers mental quiver.
