Rigging looks OK. Multiple lines from high bolts to a rigging plate. Two IDs off the rigging plate. Single person load. Stretcher looks a bit weird, something like the old VCC one that Noddy has in the shed. I imagine it would be a bit of a rough ride down. But it seems to work for them and I'd bet they'd get a lot more of those jobs than we get.

The lady involved attributes the main reason* for her injury (hit the back of her head), was a loose harness, that although cinched tight and she wouldn't have fallen out of, allowed her to continue falling backward and rotating to the side when she alleges the rope was pulling the harness upright from the attachment point, as is normal in a caught fall.
I'm not in a position to be judgemental due not knowing all the facts, however I strongly suspect she is downplaying the possibility of rotational forces that can occur in some falls due last point of contact with the rock coming into play with inertia / momentum etc. This despite not catching a leg behind the rope, which she is sure she didn't do.
(The secondary reason was not wearing a helmet.)

The rope rescue team was actually from the Cave Rescue Organisation and that was Incident #62 for 2017 from them. I was delighted to see that incident #66 was the rescue of a sheep from a pothole. The sheep was liberated, unhurt, on the fell. Velcro gloves were not mentioned.
